REGISTRATION

All four of the following documents are required for complete registration:

• Registration Form
• Physician's Medical Statement
• Contact and Insurance Information Form
• Medical Treatment Authorization and Waiver Release

 

To register for camp, campers need to send back all 4 documents along with a check for the correct amount of payment:

• $350 for overnight campers
• $300 for day campers
• A minimum of $200 non-refundable deposit due upon registration with balance of payment due by July 1, 2010
